Dental Membership Program Specifics
For Patients…
Immediate Access to the Care You Deserve - NO deductibles, waiting periods, maximums, claim forms, or pre-existing conditions!
Any patient who does not have dental insurance can purchase a dental membership. Memberships are not to be construed as insurance. All dental care must be provided by Scinico Family Dental. The program benefits are not transferable to another dental practice or specialist. It is the member’s responsibility to schedule and keep all appointments offered as part of the program. Memberships are for one year. All benefits must be used during the one year period; any benefits not utilized within the year is lost and cannot be rolled over. Memberships begin on the day of purchase. Membership dues are payable in full at the time of enrollment and are non-refundable. Cash, check or credit card payments are accepted. Members will receive 15% off any additional dental services not specifically listed as a member benefit. Payment is due on the date of service.

For Patients 13 and Younger
Routine Cleanings (2 per year)
Routine Oral Exams (2 per year)
Bitewings X-rays (Full Mouth or Panoramic X-rays Not Included)
Fluoride Treatments (2 per year)
One Emergency Exam per year & one additional x-ray (if needed)
15% discount on additional dental work.
Membership Fee: $429.00/year
Adult Plan
For Patients with Regular Dental Cleanings
Routine Cleanings (2 per year)
Routine Oral Exams (2 per year)
Bitewings X-rays (Full Mouth or Panoramic X-rays Not Included)
One Emergency Exam per year & one additional x-ray (if needed)
15% discount on additional dental work.
Membership Fee: $475.00/year
Periodontal Plan
For Patients with Periodontal (gum) Disease
Periodontal Maintenance Cleanings (3-4 per year as determined by your Dentist)
Routine Oral Exams by your General Dentist (2 per year)
Bitewings X-rays (Full Mouth or Panoramic X-rays Not Included)
One Emergency Exam per year & one additional x-ray (if needed)
15% discount on additional dental work.
